    St. Thomas Aquinas CHS - 2024-2025
    REGISTRATION OPEN HOUSE
    Thursday, February 8th, 2024
    6:00pm - 7:30pm
     
     
     
     
     
    Registration Procedures differ depending on the school your child currently attends:
     
    A. Your child currently attends St. Mother Teresa Catholic School - Russell or St. Mary Catholic School - Chesterville.
    Parents of Grade 6 students currently attending St. Mother Teresa Catholic School - Russell or St. Mary Catholic School - Chesterville, will receive a full personalized registration package at the STA Open House on Thursday February 8, 2024. Completed registration packages may be dropped off at the STA Guidance Office anytime Monday through Friday between the hours of 8:30am and 3:00pm or handed into the office at St. Mother Teresa or St. Mary. In the event you cannot make it to our open house, the registration packages will be sent home with your child the following week from school. All registration packages are due no later than March 8, 2024.
     
    B. Your child currently attends a school other that is NOT a feeder school for St. Thomas Aquinas CHS:
     
    Registration for STA for September 2024 opens Friday, February 9th, 2024
     
    1.  Parents interested in registering their child at St Thomas Aquinas CHS for Grade 7 or 8 must pre-register online at:  Registration – Catholic District School Board of Eastern Ontario – CDSBEO 
     
    2. Print and complete the required forms below. Alternatively, you may stop in to pick up a full registration package at the St. Thomas Aquinas Guidance Office starting on February 9th, 2024, between the hours of 8:00am and 3:00pm 
     
    3. Bring in your completed paperwork to the Guidance Office at St. Thomas Aquinas CHS along with the required original documents (see below) between the hours of 8:30am and 3:00pm as soon as possible.
     
    REQUIRED DOCUMENTS FOR ALL REGISTRATIONS
     
    • Student's Birth Certificate
      • If student was born outside of Canada, please complete the Confirmation of Residency and Eligibility Form below - additional documentation will be required.
    • Proof of address
      • This requirement can be met by providing a copy of either a recent property tax bill or utility bill for the address indicated on the registration form as the home address (cell phone bills will not be accepted).  Please ensure that the home address, date, and a custodial parent's/guardian's name appear on the document provided for proof of residency.
    • Student's Catholic Baptismal Certificate
      • If you child has not been baptized Roman Catholic, you must submit a formal letter to our Principal, Mr. Ralph Sharples, which includes the following: your child's name, grade, French program requested, along with the reasons you wish your child to be enrolled in our Catholic school.  If your child has not been baptized Catholic but one parent has been, please provide the parent's Catholic Baptismal certificate.                           
    • Custody
      • In the event that custodial parents are not together and there is a custody agreement in place, a copy is required for the student's file; in the absence of a custody agreement, a signed letter of acknowledgment and consent from the legal parent not registering the child will be required. 
    • Parental Consent for Release of Information
      •  Authorizing your child's current school to release information to and obtain information from St. Thomas Aquinas CHS
    • Selection of French Program Form
      • French Immersion or Core French
    • Life Threatening-Emergency-Medical-Form 
      • Required for ALL students- if no concerns, please complete student information, mark the form N/A, and sign
    • Use of Student Image Form
      • Please select one box and complete
    • Student's Most Recent Report Card (February 2023)
    • Student's Most Recent IEP (Individual Education Plan - if applicable)
    • Any other assessments/diagnoses your child may have received pertinent to their education 
     
    IMPORTANT REGISTRATION NOTES:
    1. Registrations are not considered complete until all forms and documents have been received and our administration has signed them. Uniform orders should only be placed once you have received email confirmation that your child's registration is complete.
    2. All Non-Catholic Registrations for Grades 7 and 8 must be approved by our school board and will be reviewed in the order they are submitted. Parents will be notified of decision via email.
